XC - Registration Forms - Required

These 2 forms must be completed and submitted along with a birth certificate during registration. They can be downloaded by clicking the button links on the right. They must be saved as a jpeg or pdf file to submit.

All forms must be approved by the club before your child can participate. If there's a problem with any forms you'll receive an email that they need to be re-submitted. You will receive daily reminders until they are uploaded to the site.
